Question about mailing something to Italy?

I want to mail something to a friend in Italy. Each time I have asked for his address he has given it to me but it has been written out differently each time.

Is there a proper format for address in Italy like there is in the US.
For example:

John Doe
123 Any Street
Anytown, PA 12345

basically what I am getting is
1st name Last name
Via ------
cap. ###### word
Verona Italy

does this look right ?

You can write the postal code in a number of ways and it will still get here - it could be on the same line as the city or before or after Italy. People sending me things have done it all of those and I still get the mail, but the others are right.
